Grange Park Primary School - Monday Dance Club 3:25pm - 4:30pm SUMMER TERM

Please complete this registration and payment form in full. If you have two or more siblings in this club then please complete a seperate form for each child.

Payment due

Payment is for x9 weeks of Dance After School Club. Each sessions runs at £10.

£10 x 9 = £90 from Monday 20th April - end of Summer Term Monday 6th July. Full term dates on your letter as there are some Mondays that club will not be running! Payments are non refundable.
